Terms of Vessel Survey Order Form

1. Copy of Survey Report to Marine Pandi Management Sdn Bhd.

2. Consent from Marine Pandi Management Sdn Bhd prior releasing of survey report.

3. Survey Report to be submitted to Marine Pandi Management Sdn Bhd within 7 days after Vessel is Surveyed.

4. This Survey Order Automatically Void if Survey Report is failed to submit within 45 days after SCF is issued from Marine Pandi Management Sdn Bhd.

5. Paid Fee to be refunded to Marine Pandi Management Sdn Bhd if this Survey Order is Void.

6. New Survey Order will be issue if vessel is available for survey after Termination of Survey Order.
